After booking, you'll get a text (and email) with a secure link to complete your new-patient forms from your phone.

You'll be greeted by name. Coffee, tea, or water while you settle in.

Comprehensive exam
Digital X-rays, oral cancer screening, and a thorough exam with Dr. Hamad.

Plain-English plan
We walk you through findings, options, and costs — no pressure.

Book treatment on your terms
Same-day cleanings when possible. Everything else scheduled around you.
Intake forms — sent by text
Once your appointment is booked, our office software sends you a secure link to complete new-patient paperwork from your phone. You'll receive it by text (and email as backup) — no printing, no clipboards. Didn't get it? Reply to the text or email us at info@forestdental-group.com.
What to bring
